Co-Occurring Client Process for Probation Only

CTP receives a referral from Probation Dept. for a client needing a Mental Health Evaluation (MHE) AND another Substance Abuse (SA) Treatment

Probation has chosen
1. A MHE only
OR
2. A MHE AND
ongoing sessions with the MH Counselor
as well as....
|
|
1. DUI Classes
(Ed, Ther, or Both)
OR
2. Relapse Prev
OR
3. SSC

The MH Counselor is notified of the referral and is given copies of referral and all supporting documents from probation.
|
|
Receptionist checks to see if there is an ongoing SA file already open for this client.
| YES
|
Supporting documents should be placed in the Co-Ocurr Office file for the MH counselor to review. Such as:
ASI
Client Master Assessment
Admissions Summary (if avail)
Probations supporting documents
Any other pertinent information
|
|
Client is already set up in SA group and no further action on the part of the SA counselor is necessary for this process.

|
|
|
MH Counselor decides on the appointment date & time and notifies the client and the receptionist.
|
|
Receptionist notifies the client of the need to fill out paperwork BEFORE their appt w/ MH Counselor.
|
|
Client picks up paperwork and completes it to bring back at the appointment
|

Kathy sets appt with SA Counselor approx 7-10 days later on a specific day set aside just for these appts.

By their appt date with SA counselor, the MHE from MH Counselor should be back and placed into the co-occurring office file AND
|
|
a copy should also be sent to probation
AND
placed into a file for the SA counselor.

SA counselor has appt with the client and sets them up in a the appropriate group.

|
Client is now seeing MH Counselor for mental health
AND
the SA counselor for substance abuse counseling.

|
Monthly attendance reports need to be sent to the probation dept.
