Pt can remain at UC while on vent- if they are weaned off of vent they are transferred to BI bed for 30 days or until deemed appropriate for transfer to Brooks Rehab
Pt receives 2 weeks of traditional REP (180 min/day)
Will the pt be able to be d/c home with family after 2 weeks (pt has involved family, family education is fully completed)

After 2 weeks, pt is d/c to UC to continue with modified REP program (75-90 min/day)
Pt receives 2 weeks of traditional REP (180 min/day) and family training
Pt receives 2 weeks of traditional REP (180 min/day) OR Pt receives traditional REP until determined to be medically stable (may be less than 2 weeks)









 
 
Pt remains at UC as determined by BWC or returns to Brooks Rehab if appropriate
After 2 weeks, pt is d/c home with family
After 2 weeks or when medically stable, pt is d/c to UC to continue with modified REP program (75 min/day)









** If pt emerges from UWS/MCS they will return to Brooks Rehab Hospital if deemed inappropriate for SNF -very active -need for restraints -medically unstable
 
 
 
Pt remains at UC for 30 days or as determined by insurance
