The School, Learning Community or Student Placement is notified of the student's release or future release from 30 consecutive days or more mental health placement.

Student is returning or enrolling to homeschool or last school that attended.

If general education student
|

If the student receives EC services (regular/resource)
|

If the student with a disability was previously served in a self-contained classroom or public school placement
|

|
|
|
|
|
|
|
|

|
The intervention team convenes ASAP and reviews all available information to determine how best to support the student
|
|
A multi-disciplinary team (must include student's EC Case Manager) convenes ASAP to review all information and determine how best to support the student
|
|
The Execptional Childrens Learning Community Coordinator will call the EC Placement Coordinator at 343-6960
|

Meetings and outcome decisions are documented on the Intervention Team Referral Form and in Web-IT
