If Marlise unavailable, call Bryan: (c) 215-206-1738
Marlise will explore options with referral source, based on needs of the family.
If host home found, referral source will complete: -respite referral; with overnights needed/ requested -respite consents (can be completed by respite dept if needed).
 









If introduction meeting not possible, referral source is responsible for pick up/drop off and emergency contact information will be shared over the phone
Marlise will coordinate introduction meeting in host home with respite family and referral source, if possible, where emergency contact information will be shared.
Once approved by Chester County, Marlise will fax POMS and authorization requests to Moe Stryker.
Marlise will contact Chester County to discuss needs of family and overnights being requested.
