Begin documenting problem behavior
(frequency, function, time, location)

Teacher problem solves with the student


Fill out behavioral referral form

Implement intervention based on documentation with fidelity for at least 3 weeks.

Teacher decides appropriate consequence


Call or escort student to office with referral form

If intervention is successful, no further action is needed.

If intervention is not successful...

Acknowledge student's compliance


Office staff problem solves with student

Seek assistance from colleagues, parents, PLC (Professional Learning Community), SST (Student Support Team)



Appropriate consequence given to student




Parent communication by office OR teacher.
